     - Implement Thd_ndb::is_slave_thread() which caches the value of thd->slave_thread.
 - Use Thd_ndb::is_slave_thread in execute_no_commit() and execute_commit()
 - remove now unnecessary argument "thd" to execute_no_commit() and execute_commit()
