Convert all existing code to using the new DBUG_TRACE macro, by way of a
script. This touches only code that is marked "codeformat" in .gitattributes,
so NDB remains. There are also some problematic files with explicit DBUG_LEAVE
that have been left alone (saved for the next commit).

The DBUG_ENTER / DBUG_LEAVE macros also still remain.
