This patch:
1) Removes custom rwlock implementation for Windows since all supported
   Windows versions now have TryAcquireSRWLockExclusive().
   This also means increasing _WIN32_WINNT to 0x0601.
2) Enables SAFE_MUTEX for Windows.
3) Refactors mysys mutex, rwlock and condition variable code.
