- remove function NdbHost_GetHostName()
 - remove some old windows specific hacks in NdbHost.h
 - fix copyright headers
 - remove usage of NdbHost_GetHostName() from mainAsyncGenerator.cpp
   which doesn't really need to print the name of current host
   using a portability layer function.
